Advanced Chronic Kidney Disease Program (ACKD)
Chronic kidney disease (CKD) is usually oligosymptomatic at onset, progressive and has no cure. It severely affects patients’ quality of life and generates large costs to the health care system.
Morbidity and mortality due to CKD are conditioned by socioeconomic, cultural and political factors. These generate inequalities, even within the same territory. Diabetes and arterial hypertension are the main risk factors, but CKD is also a frequent cause of complications and mortality in patients with these pathologies.
It is estimated that, for each patient in a dialysis program or for each transplant, there may be 100 cases of less severe CKD in the general population. On the one hand, these cases form the basis of patients who will later reach advanced CKD. On the other hand, these patients are at high cardiovascular risk and suffer morbidity and mortality from cardiovascular events that probably have a greater impact on health than progression to the need for renal replacement therapy.
In the Partido de General Pueyrredón, the prevalence and incidence of CKD and the main obstacles of the system for access to diagnosis and timely treatment of the disease are unknown.
The magnitude of CKD is considered to have worsened during the COVID pandemic19 due to the decline in chronic disease care in the context of the health emergency.
Program Objectives
Optimal initiation of renal replacement therapy: admission to dialysis with permanent vascular access; education and multidisciplinary support prior to dialysis.
The Renálida ACKD Unit will be made up of nephrologists, nurses specialized in nephrology, a nutritionist, a psychologist, and a social worker and will have the following healthcare objectives:
- Delay the rate of progression of CKD.
- Treat associated comorbidities that have a negative impact on the progression of CKD (poor metabolic control in diabetes, poorly controlled HT, smoking, consumption of nephrotoxic drugs, decompensated heart failure, etc.).
- Prevent uremic complications.
- To generate an optimal start to dialysis and timely referral to a transplant center.
