MODEL RENALIDA: a holistic and sustainable approach to chronic kidney disease management

Chronic kidney disease (CKD) is a rapidly growing disease that should be prioritized as a sustainable development goal. It affects 850 million people worldwide and, due to its accelerated growth, it is expected to be the fifth leading cause of preventable death by 2040. This situation is particularly critical in the population of older adults, who face a vulnerability exacerbated by risk factors such as hypertension, obesity, diabetes and sedentary lifestyles.

Since June 2022, Renálida has implemented an innovative risk management model that offers a comprehensive and systematized care plan. This model focuses especially on the PAMI population, which faces significant barriers to accessing dialysis and preventive treatments, despite the universal coverage offered by the funder. The gap between coverage and accessibility is a key challenge that the model seeks to address, through a transdisciplinary approach to early detection, characterization, efficient management and integration of the healthcare system offering efficient responses at each level of risk.

Renal Mapping is a comprehensive process that includes an initial remote assessment, detailed diagnostic studies, and a joint clinical encounter where a care plan that contributes to meaningful learning is presented. This approach engages patients in self-care by understanding their renal health status and the value of preserving it.
